normal 0 false false false en-us x-none ar-sa microsoftinternetexplorer4 background: symptoms consistent with irritable bowel syndrome (ibs) are common among patients with inflammatory bowel disease (ibd) in remission phase, and clinicians have difficulties in interpreting such symptoms as an ongoing disease activity or a coexistent ibs. we investigated if the assessment of fecal calprotectin (fc) could be helpful in this regard. methods : the study population consisted of 42 ibd patients in remission that fulfilled the ibs diagnostic criteria (rome iii), 24 ibs patients, and 30 healthy controls. clinical remission was determined based on physician’s assessments, not using corticosteroids or biological agents within the preceding six months and activity indices. the fc and c-reactive protein (crp) levels were investigated and compared among the groups. results: fc levels were significantly higher in patients with ibd (142.9 ± 216.5 µg/g) than those with ibs (24.9 ± 27.8 µg/g) and controls (17.9 ± 14.8 µg/g) (p < 0.001). crp levels were also higher in ibd than ibs patients [3.9 (se = 0.5) vs. 2.1 (se = 0.5), p = 0.030]. however, fc levels were not significantly correlated with crp levels or with severity of symptoms in ibd and ibs patients (p > 0.05). conclusions: the presence of ibs-like symptoms in ibd patients in clinical remission may reflect an ongoing activity of ibd, which is undetectable by current activity indices. serum crp levels are not specific enough in such situation, and fc is a more accurate and specific test for investigating mucosal inflammation in this regard. keywords: inflammatory bowel disease, crohn’s disease, ulcerative colitis, remission, inflammation, irritable bowel syndrome

Symptoms of irritable bowel syndrome in ulcerative colitis in remission.

The prevalence of symptoms suggestive of a motility disturbance similar to the irritable bowel syndrome was investigated in 98 patients with ulcerative colitis in remission, and a similar number of age- and sex-matched healthy controls. Thirty-three per cent of patients compared with 7% of controls fulfilled the criteria for such a syndrome (p<0.01). Contrary to expectations these symptoms were...

Managing symptoms of irritable bowel syndrome in patients with inflammatory bowel disease.

INTRODUCTION In many patients with inflammatory bowel disease (IBD), the symptoms of abdominal pain, bloating and diarrhoea are out of proportion with the demonstrated degree of inflammation; some symptomatic patients may have complete mucosal healing. Such patients often may undergo invasive, costly and potentially dangerous tests in search of residual inflammation. What are the diagnostic con...

Prevalence of Irritable Bowel Syndrome–like Symptoms in Japanese Patients with Inactive Inflammatory Bowel Disease

Background/Aims Few studies are available that have investigated the risk factors for overlapping irritable bowel syndrome (IBS)-like symptoms in patients with inactive inflammatory bowel disease (IBD). The present study has 3 objectives: (1) to assess the prevalence of IBS-like symptoms in Japanese patients with inactive IBD using Rome III criteria, (2) to examine the relationship of IBS-like ...

Calprotectin Is a Useful Tool in Distinguishing Coexisting Irritable Bowel-Like Symptoms from That of Occult Inflammation among Inflammatory Bowel Disease Patients in Remission

Background and Aim. In the inflammatory bowel diseases (IBDs), many symptoms are similar to the functional disorder irritable bowel syndrome (IBS). A challenge is thus to distinguish symptoms of IBD from IBS. The aim of this study was to investigate the levels of calprotectin in IBS-positive IBD patients in remission. Methods. Remission was defined as a simple clinical colitis activity index (S...

IRRITABLE BOWEL DISEASE Eosinophil granulocytes are activated during the remission phase of ulcerative colitis

Aim: The aim of this study was to establish a method of investigating intestinal eosinophil and neutrophil granulocytes by flow cytometry, and to compare the distribution and activity of these cells in different stages of ulcerative colitis (UC). Methods: Biopsy samples were taken from six locations of the entire colon and from the terminal ileum in 10 patients with active total UC, 10 patients...
